To change your name or other personal details on your passport, send the standard passport application form with evidence about the change.
To change your name obtain the following
Marriage/Divorce Certificate
Affidavit
Gazette
Fill in the application name and provide your new name,
Provide your previous name under the previous name section.
let your Witness sign and Stamp, and book an appointment for verification.
Scan and attach the following:
Marriage/Divorce Certificate
Affidavit
Gazette
Old Passport
