I have ever pick the form but to no avail because I couldn’t submit it with the reason that, the officer I met when I was to pick the form told me I should submit it to my employer when I asked him where it will be submitted after filling.I filled and went to the GES officer and I was told by one of them that they don’t also know the process involves.Finallu I left it with me because I didn’t know where to submit it.

Please if you could kindly educute employees about the submission processes, whether the GRA or the employer is to receive the form and if it is the employer how will they go by it
